Using the Calculator

Please be aware that this calculator is for an estimate ONLY, and insurance coverage will be determined by your insurance company based on the policy at the time of billing. Please take a moment to review our page about allowed amounts. 

Financial Agreement Amount with Provider - please use the total amount/flat rate fee in the agreement you signed (will sign) with the Provider, that you expect to pay for Midwifery services. 

Allowed Amount Source - if you do not have the allowed amounts from your insurance, use the Medicare rates that are generated, as these are often lower than your insurance allowed amount, and will help calculate your estimate.

BALANCE BILLING explained: When you see an out of network provider, they do not have a contract with your insurance company, so they will collect their whole fee, as agreed upon in your financial agreement.  If you have already paid for a service, any insurance payments will be sent to you as a reimbursement. If you have not paid for a service any insurance payments will be applied to your bill, and you will be responsible for the rest of the amount you agreed upon in your financial agreement, the 'balance.'
